Please take a moment to download the two forms above. Both forms will be required to be signed at the time of Limo or Transport Rental.
The following rules and guidelines are for the safety of you, the respect of all passengers including yourselves and others to follow, and are expected to be followed.
- Collins Limo and Transport's vehicles are kept in top condition, and it is of great importance that all renters act in a responsible manner to preserve the condition of the vehicles.
- Renter is responsible for the actions of all passengers, and assumes full financial responsibility for any damages incurred to the vehicle whether caused by accident, neglect or intent. Any such damages will be charged to Renter by Collins Limo and Transport.
- Renter acknowledges that under aged consumption of alcohol or possession of illegal drugs are strictly prohibited in any and all vehicles
- There is NO Smoking allowed in vehicles
- Collins Limo and Transport and their drivers reserve the right to refuse or terminate service without refund when and if responsible behavior of guest are not upheld.
- Collins Limo and Transport and their drivers assume No responsibility for any left articles in the vehicles at any time during or after the time in service with renter.
- Renter assumes responsibility for any overtime fees.
- Collins Limo and Transport is NOT responsible for delays or inconveniences due to traffic, unseen mechanical failures, or any situation deemed as an act of God.
- Renter authorizes Collins Limo and Transport to charge credit card on file to cover damaged cost or any other dues accumulated from the service rendered at the end of rental.
- In the event of a dispute of this agreement or any charges, the parties agree that Georgia law should apply and that such dispute shall be resolved in court of Coweta County, GA.
